How to sleep with shoulder pain and improve comfort at night
As an occupational therapist, I often hear the same concern from my shoulder patients: “I can’t sleep because of my shoulder pain.” When your shoulder hurts, even simple things like rolling over in bed or finding a comfortable position can feel impossible. The good news is that small adjustments to how you sleep can significantly reduce pain, protect healing tissue, and help you wake up feeling less stiff.
Sleep is one of the body’s most powerful tools for healing. Choosing the right sleeping position can help decrease pressure on painful structures, support proper joint alignment, and prevent awkward postures that can make shoulder pain worse overnight.

Why Sleeping Position Matters for Shoulder Pain
The impact of sleep posture on shoulder pain at night
Your shoulder is a complex joint made up of bones, muscles, tendons, and a joint capsule that all rely on proper positioning and alignment in order to stay healthy. Poor sleeping positions can:
- Increase compression in the shoulder joint
- Strain the rotator cuff and surrounding muscles
- Aggravate inflammation
- Cause morning stiffness and soreness
For conditions like rotator cuff injuries, impingement, frozen shoulder, dislocations, post-surgical recovery, or general shoulder tightness, sleep posture plays a key role in pain management.

Sleeping on Your Back: The Best Overall Option
This is often the most shoulder-friendly position as it promotes alignment of the spine, evenly distributes body weight, and prevents direct pressure on the shoulder, reducing strain on the joints and tendons.
- Place a small pillow or folded towel under the forearm/elbow of the affected shoulder to slightly elevate and support the shoulder
- Let the arm rest gently on your stomach or a pillow across your torso
- Keep a pillow under your knees to reduce lower back strain and encourage full-body relaxation
This position reduces compression and keeps your shoulder in a neutral position with supported alignment.

Side Sleeping – With Support (If You’re a Side Sleeper)
You can still sleep on your side safely with the right setup.
- Sleep on your non-painful side
- Hug a pillow in front of your chest to support the affected arm
- Let your top arm rest fully supported on the pillow rather than hanging forward
- Place a small pillow or rolled-up blanket under the armpit of the lower (supporting) shoulder to help prevent compression through the shoulder joint and improve comfort
- Place a pillow between your knees to improve hip and pelvic alignment, which can reduce strain through the upper body and shoulders
- Consider placing a body pillow beside you if you move a lot at night
This prevents the shoulder from collapsing forward and reduces stress on healing tissues.

👉 If you’ve tried these positions and still wake up in pain, it’s time for a personalized plan. Let the team at Teton Physical Therapy help you find targeted pain relief.
Reclined Position (Helpful After Surgery or for those with Severe Pain)
- Use a recliner chair or prop yourself up in bed with several pillows
- Keep your arms supported with pillows or rolled towels
- Slightly elevate your upper body to reduce pressure on the shoulder joint
For people with intense shoulder pain or post-operative discomfort, a reclined position is often more comfortable than lying flat.

Sleeping Positions to Avoid

Sleeping on Your Painful Shoulder
- Increases joint compression and inflammation
- Often leads to increased stiffness and pain in the morning
- Reduces circulation to irritated tissues
Arm Overhead Positions
- Sleeping with your arm stretched above your head can put increased strain the rotator cuff and shoulder capsule
- May increase nerve tension and cause numbness or tingling

Curling Forward Without Support
Letting your arm drop forward without a pillow can pull the shoulder into a painful, strained position

How to choose the right pillow for shoulder and neck support
The right pillow and pillow placement can make a huge difference:
- Use a firm pillow that holds its shape through the night
- Make sure your head pillow is not too high or too low, keeping your neck in a neutral position instead of bending too far to one side
- If your pillow is too low, place an object like a folded blanket underneath so the pillow sits at the appropriate height for your neck
- If your pillow is too high or too tall, consider a different pillow
👉 Small adjustments can help—but the right diagnosis makes the biggest difference.

How Long Does It Take to Notice Improvement?
Many patients begin to feel relief within a few nights of changing their sleep position. However, consistency is key. Proper shoulder support during sleep reduces overnight irritation and can significantly improve how your shoulder feels during the day.
Final Thoughts
As a therapist, I feel it is important to remind patients that pain-free sleep should be considered part of the healing process. Simple changes in sleeping position and pillow support can dramatically reduce shoulder pain, improve sleep quality, and help your body recover more efficiently.
If shoulder pain is regularly disrupting your sleep in any way, it’s time to consult an occupational therapist for a personalized treatment plan.
